Community Alternatives Program Mentally Retarded/Developmentally Disabled (CAP-MR/DD) - Medicaid offered program to help developmentally disabled individuals with intermediate level needs to remain in community settings. This program requires Medicaid eligibility and a waiting period for services to begin. CAP-MR/DD is broken down into several sub groups. The services we offer are listed below:
Home and Community Support Services - provides an individually tailored support system/and one-on-one worker which provide a goal oriented, person centered plan in order to assist the client in learning new goals and improving their skills in areas that may include: socialization skills, independent living, self care, decision making, self direction, and basic learning skills depending on the specific needs of each client.
Personal Care Services - Certified Nursing Assistants (CNA’s are not required for all CAP PC services) provide assistance with bathing, dressing, hygiene, grooming, and many more activities of daily living.
Respite Care Services - is a service that provides temporary relief for a family member or caregiver. Respite Care services can be arranged to meet a variety of needs including: family vacations, hospitalizations, family emergencies, crisis, business trips, and basic relief of duties for a person’s primary caregiver due to stress, or sickness. Respite Care can be provided on an as needed basis in order to meet the family/caregiver and client needs as long as the hours have been authorized for use.
Enhanced Personal Care/ Enhanced Respite Care Services - Enhanced services are for persons who require more intense medical or behavioral needs. North Carolina Outreach Home Care, LLC. will provide staff with the appropriate medical training and registered nurse consulting and supervision as needed.
Day Supports – Facility-based service that provides assistance with acquisition, retention, or improvement of self-help, socialization and adaptive skills. Meets the day programming needs of individuals who choose to attend or receive services provided by a licensed facility.
Home Supports – This service meets the habilitative and personal care needs of adults who choose to live with their family in the family home and receive supports from their family. Provides assistance with acquisition, retention, or improvement of skills related to activities of daily living.
Residential Supports (Alternative Family Living) – This service provides assistance with acquisition, retention, or improvement in skills related to activities of daily living necessary to enable an individual to reside in a non-institutional setting. These services can be provided in licensed community residential settings, foster homes, as well as unlicensed alternative family living homes.
